I, _______________, hereby grant _______________ and its production partner, Foreword, including their affiliates, successors, and assigns, the irrevocable and unrestricted right to use and publish photographs or video of me, or in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium; and to alter the same without restriction. I hereby release _______________ and Foreword and their respective legal representatives and assigns from all claims and liability relating to said photographs or video. I am of full legal age and have read this release and am fully familiar with its contents.
